Methylphenidate XL tablets are extended-release formulations of methylphenidate hydrochloride designed for once-daily dosing in the treatment of attention deficit hyperactivity disorder (ADHD) in adults and children aged 6 and over. The XL formulation delivers methylphenidate in a biphasic pattern: an initial immediate-release phase providing rapid onset of therapeutic effect, followed by a sustained-release phase that extends symptom coverage throughout the day. A single morning dose provides effective ADHD symptom management for up to eight to twelve hours, avoiding the need for multiple daily doses. Methylphenidate XL is a Schedule 2 controlled drug available only on specialist prescription at Pharmacy Mentor.
How It Works
The extended-release coating or matrix system of methylphenidate XL tablets delivers the dose in two phases. Approximately 50% of the dose is released immediately after ingestion, producing the initial rapid therapeutic effect. The remaining 50% is released gradually over the following hours, extending the duration of action and maintaining therapeutic plasma levels throughout the working or school day. As with the immediate-release formulation, methylphenidate blocks reuptake of dopamine and norepinephrine, improving attention, impulse control, and executive function in people with ADHD. Tablets must be swallowed whole, crushing or chewing disrupts the extended-release mechanism and results in rapid uncontrolled dose release.
Why Once-Daily Dosing is Often Preferred
The once-daily dosing schedule of methylphenidate XL eliminates the need for midday doses at school or in the workplace, removing the logistical and social challenges associated with multiple-dose regimens. This is a significant practical benefit for many patients: both children who would otherwise require medication to be administered at school, and adults managing busy schedules who may find midday dosing difficult to maintain consistently. Once-daily dosing also generally supports better treatment adherence and more predictable day-to-day symptom control.
Available in 18mg, 27mg, 36mg, and 54mg strengths, methylphenidate XL allows precise dose titration under specialist supervision from a low starting dose through to the individually effective therapeutic dose for each patient. Regular clinical review during the titration phase ensures that the optimal balance between symptom improvement and tolerability is achieved.
Important Note
Methylphenidate XL is a Schedule 2 controlled drug and must be prescribed by a clinician with specialist ADHD expertise. Tablets must be swallowed whole and must not be crushed, chewed, or split. It is contraindicated in patients with hyperthyroidism, pheochromocytoma, glaucoma, significant cardiovascular disease, or those using or having recently used MAOIs within 14 days. Do not take in the afternoon or evening as this may cause insomnia. Regular monitoring of blood pressure, heart rate, and clinical response is required. This medication must only be used as directed by a qualified specialist prescriber.
