Elvanse contains lisdexamfetamine dimesylate, a central nervous system stimulant prodrug used in the treatment of attention deficit hyperactivity disorder (ADHD) in adults for whom a previous adequate trial of methylphenidate has been considered clinically inappropriate or has not provided sufficient symptom control. Taken as a once-daily capsule, Elvanse provides up to fourteen hours of ADHD symptom coverage from a single morning dose. How It Works

Lisdexamfetamine is an inactive prodrug that requires enzymatic conversion to its active form, d-amphetamine (dexamfetamine), after gastrointestinal absorption. This conversion occurs primarily via red blood cell enzymatic activity and produces a gradual rise in active amphetamine plasma concentration, providing a smoother onset of effect than direct amphetamine formulations. D-amphetamine then acts to increase the release and block the reuptake of both dopamine and norepinephrine in presynaptic neurons, producing enhanced dopaminergic and noradrenergic activity in neural circuits critical to attention, executive function, and impulse control. The prodrug mechanism limits the rate of rise in active amphetamine concentration, which reduces the reinforcing effects that contribute to misuse potential compared to direct amphetamine preparations.

Why Elvanse is Used in ADHD

Elvanse is indicated for adults with ADHD who have not responded adequately to methylphenidate preparations or in whom methylphenidate is not clinically appropriate. Elvanse is a Schedule 2 controlled drug and must be prescribed by a clinician with specialist ADHD expertise. In the UK, Elvanse is licensed for the treatment of ADHD in adults only — it is not licensed for children in the UK. Elvanse is contraindicated in patients with symptomatic cardiovascular disease, severe hypertension, advanced arteriosclerosis, hyperthyroidism, glaucoma, history of substance misuse, agitated states, and current or recent MAOI use within 14 days. Common side effects include decreased appetite, insomnia, dry mouth, headache, and increased heart rate and blood pressure. Cardiovascular monitoring is required throughout treatment.
