Concerta XL uses the proprietary OROS (Osmotic Release Oral System) delivery mechanism, which delivers methylphenidate at a controlled, ascending rate over approximately twelve hours from a single morning dose. This unique delivery system replicates the clinical effect of three separate immediate-release doses across the day while providing the convenience of once-daily dosing. How the OROS System Works

The Concerta XL tablet has an immediate-release outer coating containing 22% of the total methylphenidate dose, which dissolves quickly after ingestion to provide a rapid initial therapeutic effect. The remaining 78% of the dose is contained within the osmotic tablet core, from which methylphenidate is released at a gradually ascending rate over approximately ten to twelve hours as water enters the tablet through a semi-permeable membrane, driven by osmotic pressure. This ascending delivery profile means that the highest plasma concentrations of methylphenidate occur later in the day, providing increasing coverage during the afternoon when cognitive demands are typically high. The intact tablet shell, which remains after the drug is released, may be visible in the stool — this is normal and expected and does not indicate that the medication has not been absorbed.

Important Note

Concerta XL is a Schedule 2 controlled drug and must be prescribed by a clinician with specialist ADHD expertise. Tablets must be swallowed whole — they must not be crushed, chewed, or split as this destroys the OROS delivery mechanism. The tablet shell visible in the stool is normal and expected. Concerta XL is contraindicated in patients with hyperthyroidism, pheochromocytoma, glaucoma, significant cardiovascular disease, and concurrent or recent MAOI use. Regular cardiovascular monitoring is required. This medication must only be used as directed by a specialist prescriber.
