How It Works
The modified-release bead system delivers methylphenidate in two phases. Immediate-release beads dissolve rapidly after ingestion, providing an initial rise in plasma methylphenidate concentration within 30 to 45 minutes. Extended-release beads are coated to delay their dissolution, releasing methylphenidate gradually over the following hours to maintain therapeutic plasma levels and symptom coverage. This provides a duration of action of approximately eight hours from a single morning dose. If the capsule is opened for sprinkling onto food, the beads must not be crushed or chewed as this disrupts the modified-release coating and destroys the sustained-release mechanism, resulting in uncontrolled rapid dose delivery.
The Sprinkle Option as a Clinical Advantage
The ability to open the capsule and sprinkle the beads onto food makes methylphenidate M/R capsules particularly suitable for younger children or other patients who have genuine difficulty swallowing capsules or tablets. The beads can be added to a small spoonful of soft food such as applesauce and swallowed without chewing, providing a clinically equivalent dose without the requirement to swallow a whole capsule. The full capsule contents must be used immediately after opening — the beads must not be stored or pre-prepared in advance.
